Thank you for completing this online Prospective Charter report. Each semester, every active chapter and Prospective Charter is required to submit a report of the changes in membership status of its members, among other operational information. While Prospective Charter members are not assessed individual per capita tax, we do require membership updates to be submitted each semester, as well as the remittance of the assessed $160 convention tax for Prospective Charters in Phases II and III.

The report that you will find on the following pages has been specifically compiled for the current Prospective Charters. You will be asked to provide a current listing of all Prospective Charter members, financial status, and operational responses. You can use the provided “Save and Continue Later” button to return to a saved form for a period of 30 days.

This report is required to be submitted, paid, and completed by November 15 and April 15.

For Phase II and Phase III Prospective Charters only: Once this report is submitted with accurate information, the Prospective Charter President or Treasurer will be required to remit the $160 convention tax by electronic check or card payment, or mail a physical check to the National Headquarters at the following address:
